The quality and nutritional value of our meals is paramount to our service. Further, it's important to provide a diverse diet to our residents. This is both to respect cultural preferences as well as ensuring residents maintain a healthy intake of nutrition. That's why, in this role, you will work closely with the Cook and Nursing staff. Meal Preparation Practices include:- Assisting in preparation of meals which cater for the personal preferences, cultural differences and changing health status of residents, consistent with current nutritional standards
- Assisting in the preparation of meals according to the four week rotating menu with the cook as required
- Assisting in the preparation of meals for birthdays, special events or functions as determined by management
- Preparing drinks and special fluids as necessary for individual residents
- Preparing meal trays/tables with consideration for the residents individual needs and dignity
- Assisting with meal plating. This includes ensuring the presentation, quantity and temperature of food served is appropriate for the resident’s dietary requirements
- Assisting with the delivery of meals and the collection of meal trays
- Dismantling meal trays, clearing and preparing dishes and cutlery ready for the dishwasher
- Maintaining clean food preparation areas, cooking equipment, food storage areas, cupboards and utensils as detailed on the kitchen cleaning list
- Practicing all food handling practices in accordance with all Food Handling Legislation/Regulations especially in relation to

The primary responsibility of this role revolves around the medication administration issues for older people. Therefore, you will provide the delivery of skilled, individualised nursing care of the highest possible standard for all our residents. Overall, we adhere strictly to the standards set down by the Department of Health and Ageing at all times. Some key duties of this role include:- The nutrition and hydration maintenance of every resident and reporting any changes in a residents appetite to the sister in charge
- Ensuring individual continence management programs are attended to at all times. In fact, they must be assessed, evaluated and changed as necessary
- Ensuring pressure area care is provided according to our residents' individual routines
- Ensuring mobility and dexterity is maintained for every resident by implementing physiotherapy plans when the physiotherapist is not available
- Respecting cultural instructions and ensuring they are continued for residents
- Ensuring residents are prepared in time for their individual activities and therapy plans. These are stated on the allied health and diversional therapy program and listed at each nurse's station
- Ensuring residents are prepared in time for outings as may be organised by us or their relatives & friends
- Ensuring complete and satisfactory care of a dying resident is attended to when necessary. This is done by following policy guidelines as outlined in the aged care palliative care seminar
- Performing cardiopulmonary resuscitation according to the staff members level of expertise, or ensuring capable staff are immediately alerted if the situation occurs where it is required
- Observing and reporting to the Unit Manager or sister-in-charge any important changes in a resident's behaviour or condition
- Documenting any changes in a residents' behaviour or condition for future reference
- Assessing residents for care problems to be included on the nursing care plan
- Implementing all care as listed on the nursing care plan, and including on the care plan any changes in residents' problems requiring further interventions

- You will provide residents with skilled, individualized nursing care of the highest possible professional standard based upon currently researched and accepted gerontological nursing practices, including areas of:
- Wound Management
- Pain Management
- Continence Management
- Medication administration issues for older people
- Restraint Management
- You will help make sure that the philosophy, objectives and policies of Vermont Aged Care are upheld at all times, with due regard to resident and organisation needs
- You will help make sure that the Aged Care Accreditation Standards as set down by the Department of Health and Ageing are met at all times
- You will help make sure that all relevant resident documentation is completed by nursing staff on their shift
- You will help make accurate evaluations of care activities are attended to and that appropriate further care is implemented where needed
- You will help with the prioritising and appropriate sequencing of care activities is undertaken by you and other staff
- You will help make sure that the staff know and implement Universal Precautions and infection control principles
- You will help with the correct dispensing of medications to residents either by themselves or another Registered Nurse, Division 1 or higher
- You will help make sure that pharmaceutical items are ordered for residents as prescribed by medical practitioners, medication charts are current, legible and accurately completed by medical practitioners at all times
- You will help make sure that residents’ medical practitioners and family are informed immediately of any change in a resident's condition
- You will help make sure that the amenities are organised and coordinated for all residents (i.e. hairdressing, podiatry, physiotherapy) as necessary
- You will help with the assessment of the performance of staff under your supervision is undertaken each shift. Further, you will ensure professional standards of care are maintained at all times. Also, you will ensures adequate ward stock is available at all times to enable the delivery of optimum care in a cost effective manner
What about ongoing learning and my education?
We will support you in the maintenance of clinical knowledge and skills in Gerontic Nursing care. For example, you will be attending regular in-services and information sessions. Further, on some of your shifts you will attend regular lecture sessions as provided by one of our staff of off-site specialists. Better still, off-site training and events may be provided to you as required. - Ensures medical staffs are liaised with as necessary, coordinating regular visits from all Allied Health Professionals as necessary

- Ensures complaints by residents are dealt with immediately by the sister-in-charge until a satisfactory solution has been found (and where this has not been possible, would advise the Director of Nursing for further action and achievement of a satisfactory solution)
- Ensures all legal documentation requirements are upheld at all times and promoted to all staff.

Nursing assistants & Personal Care Assistants are responsible for helping to ensure the quality care our residents deserve. - The nutrition and hydration maintenance of every resident. This includes reporting any changes in a resident's appetite to the Registered Nurse (RN) in charge

- Ensuring pressure area care is provided according to a residents individual routine
- Ensuring mobility and dexterity is maintained for every resident by implementing physiotherapy plans when the physiotherapist is not available.
- Respecting any cultural instructions and ensuring they are continued for residents
- Ensuring residents are prepared in time for their individual activities and therapy plans stated on the allied health and diversional therapy program. You will find these listed at each nurses station
- Ensuring residents are prepared in time for outings as may be organised by the aged care or relatives and friends
- Ensuring complete and satisfactory care of a dying resident is attended to when necessary as per policy guidelines
- Performing cardiopulmonary resuscitation according to the staff members level of expertise or ensuring capable staff are immediately alerted if the situation occurs
- Observing and reporting to the Unit Manager or RN-in-charge of any changes in a resident's behaviour or condition
- Documenting any changes in a resident's behaviour or condition as directed
- Assessing residents for care problem, to be included on the nursing care plan, and reporting these
- Implementing all care as listed on the nursing care plan
- Documenting on-going evaluations of nursing care plans and other relevant resident documentation as listed in the documentation policy, or as directed by the RN in charge or Director of Nursing (DON)
What other tasks are NA/PCA responsible for ensuring:
In the area of Teaching and Education, as well as Quality Systems involvement:- Maintenance of aged care knowledge and skills in aged care by attending regular inservices and information sessions
- Information, journal articles etc. provided by the aged care are read and the information is incorporated in to work practices as required
- A continuing commitment to further education is promoted and followed
- Harmony and a team approach exists as a culture between staff
- Assistance in quality improvement exercises is provided to the Director of Nursing and Registered Nurses as required performing other such duties as they are capable of as requested by the Director of Nursing or Registered Nurse in charge
- A safe working environment is maintained for staff at all times. This is promoted following the guidelines as set in the aged care’s policy manual (ie. correct minimal-no lifting procedures, faulty equipment is reported immediately...)
